Service Coordination Program
The Service Coordination Program provides services for elderly and disabled
public housing residents that will allow them to maintain the independence of
living in their own homes. The goal is to help elderly people remain in their
own homes as long as possible and live with dignity.
Service Coordinator Case Managers are responsible for establishing service
needs, determining eligibility for services and assisting with funds needed for
payment of services.
Examples of services provided to participants include: homemaker-household
chores, meal preparation, transportation, home health care, legal service referral,
supportive devices, and prostheses, recreation-social opportunities, volunteerism
and adult day care. Additional services can be provided on an individual basis.
Eligible participants must live in public housing. They must be at least 62
years old or disabled. There is no charge for any services provided by the Service
Coordinator Program.
